Four Medicare programs
One loop. Four ways to bill it.
Your patients already qualify for more than you are running. The difference between these programmes is who counts and what the month has to show, not how the work gets done.
Your team reviews. Your provider signs.
Which programmes fit this patient?
Describe the patient. The programmes light up.
Toggle what is true of one patient. Every programme they could be considered for stays lit; the rest step back. Leave everything on Any and this is simply all of them.
The patient
Programmes to consider
All 4 lit
Any patient on your panel. Describe one on the left.
Can they stack?
What one month can hold for the same patient, and what it adds up to.
- CCMBHI$123.58in one month
- APCMBHI$111.23in one month
The same minutes never count twice, whichever codes the month carries.
Not by the same practitioner in the same month: CCM and APCM; CCM and PCM; APCM and PCM; BHI and PCM.
A lit row means a patient like this can be considered for that programme, not that they are eligible. Consent, the initiating visit and who may bill are covered on each programme page, and this cannot check them for a patient. A stack is what may be billed together, not what this patient qualifies for. Programs are what your practice bills. HANA does not bill, and its call time is never counted as clinical time. Requirements here are the Medicare rules as they stand in October 2026 and are worth checking against your MAC before you rely on them.
How it works
Forty-five minutes a patient.
We take it under thirty.
One coordinator. Same hours. Two hundred and fifty patients instead of a hundred and fifty.
Hana calls · you set the protocol
It calls from your number until someone picks up. Then medications, symptoms, and what changed.
Hana routes · your team decides
Anything clinical goes to your team, with the reason and the transcript attached.
Your clinician reviews · Hana writes
The note is in the chart before your team opens it. Under that patient, not in a spreadsheet.
You submit · Hana supplies the evidence
Every minute attributed to the person who earned it, ready to attest on the first.
You set the escalation rules. A person on every clinical flag, an audit trail on every call, minutes totalled per patient.
45-60min
per patient, per month, today
What a coordinator spends on one enrolled patient when the calling, the chasing and the note are all done by hand.
120-160
the caseload that caps at
Which is why most programs stall well short of what the panel could support.
29min
what HANA is built for
Same coordinator, same hours, toward 250 patients. HANA makes the calls; your team reviews and attests.
Your team reviews it. Your provider signs it. Nothing is billed until a person on your team approves it.
Questions practices ask
Before you pick one.
Whichever fits the patients you already have. Describe a patient in the filter above and the programmes they could be considered for stay lit. Chronic care management has the widest eligibility, and advanced primary care management counts no minutes at all, but that is a fact about panels, not a recommendation.
Often, and the filter above works it out for you: describe the patient and it shows the sets CMS lets a practice bill in the same month, with the combined figure. Behavioral health integration can run alongside chronic care management or advanced primary care management. Most of the other limits are about the practitioner, not the patient: the practitioner billing advanced primary care management cannot also bill chronic or principal care management that month, and one practitioner cannot bill both chronic and principal care management, though a specialist can bill PCM for a different condition while the primary care practice bills CCM. The same minutes never count toward two codes.
No. It is the same loop every month: reach the patient, flag what matters, write it up, bill it. What changes is who qualifies and what the month has to show. HANA makes the calls and writes them up in every one of them, and your team reviews and attests in every one of them.
No. They are your patients and it is your claim. HANA supplies the reach and the documentation, your team supplies the clinical judgement and the attestation, and nothing bills until a person on your team approves it.
Not today. Remote physiologic and therapeutic monitoring turn on a device recording readings at home, and HANA does not supply devices. The four programmes on this page need none.
Have a chat with HANA.
Give us your number. It calls in about ten seconds. Talk to it like a patient would, then let your team try it.
Hear Hana handle a real patient conversation.
Enter your details. Hana texts you to confirm, then calls within seconds, so you can hear it for yourself.
